Inspector Notes

An unannounced renewal inspection was conducted on 1/4/2024 and 1/5/2024. There were 60 children, ages 3 months-5 years, 18 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed during the inspection. A total of 5 children?s records, 5 staff records and 6 medications were reviewed. The children were observed during center time and lunch. The inspector arrived for the inspection at 9:40am and departed the center at 11:42pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and discussion with staff, the center failed to ensure prepared infant formula shall be date and labeled with the child?s name.

Evidence:
1. Licensing staff observed a bottle with formula on the counter by the dish drainer that contained approximately 1-2 ounces of formula and was not dated or labeled with a child?s name. Staff 1 stated ?the label was removed so the bottle could be washed?.

2. Licensing staff observed Staff 1 feeding a bottle to Child 1 that was not dated or labeled with a child?s name and Staff 1 stated that ?the child pulled the label off?.
Plan of Correction: Provider will ensure staff date and label all prepared infant formula bottles.
