Inspector Notes
An unannounced, on-site renewal inspection was completed on 09/12/2024. The on-site inspection began at approximately 9:15 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 5 children and 2 caregivers present. The inspector reviewed 5 child records and 2 caregiver records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/25/2024. A POC submitted after this date will not appear on the public website.
Violations
5Evidence:
A 3-month-old infant was placed in a crib located in a room out of the sight of the caregiver to nap while the caregiver provided care to children in another room.
Based on record review, the provider did not ensure children's records contained all required information.
Evidence:
The record for C3 did not contain the name, address, and telephone number of a second designated person to contact in case of an emergency if the parent can not be reached.
Evidence:
The provider failed to wash C1?s hands after a diaper change.
Evidence:
S1 reported not being able to locate documentation of the annual review of the emergency plan.

Licensing inspector did not observe a menu posted for the current one-week period.
