Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 5/24/21 and concluded on 5/28/21. The provider was contacted by telephone to initiate the inspection. There was children present with the provider for a total of 3 points. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 provider/household member records submitted by the provider to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law in the area of administration and violations were documented on the violation notice issued to the provider.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations
1Evidence:
1. The most recent tuberculosis test for the provider is dated 4/17/19. The provider was due for an updated TB screening or test in April 2021.
2. The most recent tuberculosis test for household member 1 is dated 4/29/19. Household member 1 was due for an updated TB screening or test in April 2021.
3. The provider confirmed that the TB tests for herself and household member 1 are overdue.
