3Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for child 1, who had been in care for three months.
Plan of Correction: The provider responded with the following: Child 1's physical was obtained upon enrollment; however, the parent needed the record and had not yet returned it to the provider. The provider will be contacted today to remind them to bring a copy of the physical back to the provider.
Standard 8VAC20-800-90-A
Before attendance, documentation of immunizations is to be obtained for each child. There was no documentation of immunizations for child 1, who had been in care for three months.
Plan of Correction: The provider responded with the following: Child 1's immunization record was obtained upon enrollment; however, the parent needed the record and had not yet returned it to the provider. The provider will be contacted today to remind them to bring a copy of the immunization record back to the provider.
Standard 8VAC20-800-960-B
Infants who cannot hold their own bottles shall be picked up and held for bottle feedings. Bottles shall not be propped. During the inspection, there was an infant in a carrier seat drinking out of bottle that was propped up by a blanket.
Plan of Correction: The provider responded with the following: All caregivers will ensure that infant bottles are not propped up and that if an infant cannot hold a bottle themself, they will be held and fed by a caregiver.
