An unannounced monitoring inspection was conducted today.
There were 13 children present with two staff supervising.
The children were playing in the gym.
Two child records were reviewed on site. Two staff records were reviewed at the main office.

Based on a tour of the playground, the facility failed to ensure that that all equipment is maintained in safe condition.
Evidence:1. On the new climber, where the slides attach to the deck, there are gaps of 1/4 inch. This presents and entanglement point.
Plan of Correction: The program manager will ensure that the bolts on the slides are tightened to close up the gap.
