Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on and concluded on 09/30/2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 33 children present, ranging in ages from 3 to 5 years old, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the Religious Exempt program did not ensure that a Federal Fingerprint Criminal Records Search is conducted for each staff member prior to their first day of employment.
Evidence:
The records for 3 staff were reviewed during the inspection.
Documentation submitted did not include results of a Federal Fingerprint Criminal Records Search for staff #1 who has been employed since 9/8/2021.
The program director stated that she forgot to send staff #1 to have the fingerprints taken at the time of hire.
Plan of Correction: The program director has had staff #1 make an appointment for fingerprinting next week and will forward the results of the criminal records search to the inspector as soon as it is received. Future new staff will have a criminal records search conducted prior to their first day of employment.
