An unannounced monitoring inspection was conducted on April 30, 2024. This on-site visit was conducted from 9:50 a.m. through 10:35 a.m. Seventeen children, ages 3-5, were in the care of three staff. Five children and five staff records were reviewed.

Evidence:
1. Five staff records were reviewed.

3. The director confirmed an out of state central registry check had not been obtained for Staff #1.
