Inspector Notes
An unannounced, on-site code compliance inspection was initiated on 05/12/2025 and completed on 05/20/2025. The on-site inspection began at 10:06 AM and ended at 11:40 AM. The inspector reviewed compliance in the areas listed above. There were 17 children present with 3 staff. The inspector reviewed 5 children?s records and 5 staff records onsite.
This inspection included a document review, a tour of the facility, and interviews. Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.

The center did not request a search for the child abuse and neglect registry and criminal from two states for Staff #4, who has been employed for more than 2 years and was working with children at the time of the inspection.
