Inspector Notes
A subsidy health and safety inspection was conducted on October 27, 2023 from approximately 10:30 am to 12:30 pm. There were 45 children present, ranging in ages from 3 months to 4 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 6 children's records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, and interview, the center failed to ensure that staff submit subsequent TB screenings at least every two years from the date of the initial screening.
Evidence:
1. The record for staff #2, date of employment 04/16/12, contained a TB screening dated 09/21/2021. A subsequent TB screening was due on or before 09/21/2023.
2. The record for staff #3, date of employment 09/02/2014, contained a TB screening dated 08/21/2021. A subsequent TB screening was due on or before 08/21/2023.
3. The record for staff #4, date of employment 11/23/2020, contained a TB screening dated 11/17/2020. A subsequent TB screening was due on or before 11/17/2022.
