Inspector Notes

An unannounced inspection was initiated on 11/15/2024 and completed on 11/15/2024 in response to a complaint received by the licensing office on 10/30/2024 relating to allegations of ratios. This inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/25/2024. A POC submitted after this date will not appear on the public website.

The child day center employs supervisory personnel according to one staff member to four children from ages zero to 16 months.

On 11/15/2024 at approximately 8:22am, 13 children were observed ages 6 months through 18 months under the care of three staff. The center did not maintain ratios of 1 staff for every 4 children. At approximately 8:30am, a 4th staff arrived.
Plan of Correction: We have changed the hours to 2 employees. One from 8:30 to 7:30 and the other from 8:00 to 8:15.
