Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.
A code compliance inspection was initiated on 02/17/2021 and concluded on 02/17/2021. The director was contacted by video call to initiate the inspection. There were 15 children present and 2 staff. The Inspector reviewed 4 staff records and 2 children?s records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations

1. The record of staff #1 (start date: 8/16/21) had documentation of fingerprint based national criminal history search results dated 9/20/21.
2. The record of staff #2 (start date: 8/16/21) had documentation of fingerprint based national criminal history search results dated 9/17/21.
3. The record of staff #2 (start date: 5/1/21) had documentation of fingerprint based national criminal history search results dated 9/14/21.
4. The center used favorable background checks received during previous employment with the center for staff, however the staff members were not employed with the center for more than a year. In those instances, staff should be treated as new hires and new background checks must be obtained.
Evidence:
1. The record of staff #1 (start date: 8/16/21) did not have documentation of central registry results and had documentation of a completed sworn statement or affirmation dated 10/3/21. The center director stated that they mailed off the completed central registry form for staff #1 in the beginning of February.
2. The record of staff #2 (start date: 8/16/21) had documentation of central registry results dated 10/30/2021 and sworn statement or affirmation dated 9/16/2021.
3. The record of staff #3 (start date: 1/5/22) did not have documentation of central registry results. The center director stated that they mailed off the completed central registry form for staff #1 in the beginning of February.
4. The record of staff #4 (start date: 5/1/21) had documentation of central registry results dated 10/30/21 and completed sworn statement or affirmation dated 9/23/21.
Central Registry forms will be submitted 7 days before hire.
