Inspector Notes
A code compliance inspection was conducted on Wednesday, April 26, 2023. The inspector was on site from 10:15am to approximately 11:25am. There were 42 children in attendance with nine staff directly supervising. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, hallways, outdoor play areas, and bathrooms were inspected. Five children's records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: 1) The staff health report in the record of Staff #1, employed 06/01/22, was completed on 10/11/22. 2) During interview, Staff #1 acknowledged the staff health report was completed after employment.
Evidence: During the inspection, nine staff were observed supervising children. Management reported that staff observe children upon arrival for signs of illness, but the staff did not have current certification in conducting daily health screenings when the inspection was conducted.
Evidence: 1) The results of the fingerprint-based national criminal background check in the record of Staff #1, employed 06/01/22, were dated 03/21/23.
2) The results of the fingerprint-based national criminal background check in the record of Staff #2, employed 08/29/22, were dated 03/23/23.
3) The results of the fingerprint-based national criminal background check in the record of Staff #4, employed 10/26/21, were dated 10/27/21.
4) The results of the fingerprint-based national criminal background check in the record of Staff #5, employed 08/02/21, were dated 08/23/21.
5) During interview, management confirmed the fingerprint-based national criminal background check for Staff #1, Staff #2, Staff #4, and Staff #5 were not obtained prior to employment.
Evidence: 1) The results of the central registry check in the record of Staff #1, employed 06/01/22, were dated 07/14/22. The sworn statement was completed on 08/15/22.
2) The results of the central registry check in the record of Staff #2, employed 08/29/22, were dated 11/19/22.
3) The results of the central registry check in the record of Staff #4, employed 10/26/21, were dated 01/31/22.
4) The sworn statement in the record of Staff #5, employed 08/02/21, was completed on 08/19/21.
