Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/15/24, as a part of the licensure period. The on-site inspection began at 2:55 PM and ended at 3:45 PM. The inspector reviewed compliance in the areas listed above. There were 41 children present and 4 staff. The inspector reviewed 4 children?s records and 4 staff records (specific items only) on-site
This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Child 1's record listed a parent as an emergency contact.
Child 2's was missing complete addresses for both emergency contacts.
Child 2's record was missing a parent signature for the annual update.
