Inspector Notes

A complaint inspection was conducted on March 7, 2022, concerning the maintenance of the facility. An onsite inspection was conducted of the facility. The complaint was deemed valid. Violations related to the complaint as well as other violations observed during the inspection are noted on the violation notice. Standard 8VAC20-780-270-A
Based on observation areas of the center, inside and outside, are not being maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping.
Evidence
Peeling and chipped paint on the walls was observed in the hallway, both the infant rooms, the preschool room, 2b classroom, school age room and the 4's classroom. The playground has a missing piece of the wooded fence and splintering of the wood of the bottom. The playground has paper trash all around, a plastic glove, a bag of trash, mittens and other objects. The front left side of the facility has an exposed broken trash dumpster that has loose trash as well as trash bags all around it.
Plan of Correction: Trash has been picked up and will continually be cleaned. A work order has been submitted to replace broken fence and to repair walls/paint. A new trash can has replaced the previous broken dumpster.

Tummy time for 30 minutes a day for Infants, who cannot turn themselves over and are awake is not being recorded.
Evidence
A review of infants daily activity who cannot turn themselves over did not have recorded tummy time for 2 or the 3 days reviewed.
Plan of Correction: Teachers will record all tummy time for infants in the kinderCare App. Tummy time chart will also record/track infants tummy time.

Based on observation and discussion with staff the diapering surface is not being cleaned with soap and at least room temperature water and sanitized after each use.
Evidence
The diapering surface is only being cleaned with an approved cleaning solution, it is not being sanitized after each diaper change.
Plan of Correction: Staff will be retrained on how to properly clean and sanitize surface at next staff meeting.
