Inspector Notes

A complaint was received in the licensing office regarding behavioral guidance, diapering and ratios. An inspection was conducted today between 9:15am and 10:30am. The inspector reviewed attendance records, policies and procedures, and written reports of children's incident/accident reports. The information gathered during the inspection determined no violations with applicable standards or law. The complaint is deemed not valid. Based on review of written records of children's serious and minor injuries, not all records contained the required documentation.
Evidence
Two out of six written records of children's serious and minor injuries did not contain two signatures. Staff member and parent or two staff members.
Plan of Correction: For parents who don't sign the incident report is to have another staff member who witnessed the incident to sign the report.
