Inspector Notes
A complaint was received by the licensing office on May 8, 2024, in regard to staff-to-children ratio requirements. An investigation was initiated on May 9, 2024, between the hours of 7:24am and 10:45am and concluded on May 28, 2024. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Areas of noncompliance not related to the complaint but identified during the course of the investigation can also be found on the violation notice. 7Based on record review, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #1 (date of enrollment 03/05/2024) did not have an immunization record on file available for review.
Based on record review, the center did not obtain documentation of a physical examination before the child's attendance or within 30 days after the first day of attendance.

Based on observation, the center failed to have the required staff-to children ratios whenever children were in care.
Evidence:
1. On 05/09/2024 The Discovery Preschool classroom (2 year olds) was out of ratio from approximately 7:36am until 7:58am. There were 3 children under the age of two present. Staff #1 was alone and out of ratio with:
-6 children from approximately 7:36am -until 7:42am
-11 children from approximately 7:42am until 7:44am
2. Staff #7 entered the room at approximately 7:44am.
3. Staff #1 left the room at approximately 7:50am.
4. Staff #7 was alone and out of ratio with 14 children from approximately 7:50am until 7:58am. Staff #1 returned to the classroom at approximately 7:58am.
Altered teacher arrival times to adequately cover ratios.
Evidence:
1. On 5/9/2024 Staff #7 was alone in the Discovery Preschool classroom (2 year olds) and out of ratio from approximately 7:58am-8:05am with 10 children.
2. Staff #5 arrived in the classroom at approximately 8:05am.
Altered teacher arrival times to adequately cover ratios.
Evidence:
1.On 05/09/2024 Staff #3 was alone and out of ratio in the Preschool classroom (3 year olds) from approximately 7:42am until 8:20am with:
-12 children from approximately 7:42am until 7:45am.
-15 children from approximately 7:46am until 7:49am.
-17 Children from approximately 7:53am until 8:20am.
2.On 05/09/2024 Staff #2 was alone and out of ratio in the Pre-K classroom (4 - 5 year olds) with 12 children from approximately 8:28am ? 8:30 am. Staff #9 arrived at approximately 8:30am.

Based on observation, the diapering surface was not used only for diapering or cleaning children.
Evidence:
1. The diapering surface in the Discovery Preschool class was used to store a laptop and a clip board during the time of the inspection.
