During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on December 4, 2024. The on-site inspection began at 9:30am and ended at 11:37am. The inspector reviewed compliance in the areas listed above. There were 52 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 11, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-B
If the child has had a physical examination prior to attendance, it shall be within 12 months prior to attendance for children two years of age through five years of age.

The record for child #1 (age 3 years) contained a physical examination dated two years and eight months prior to attendance.
Plan of Correction: Parents were notified and set up a doctors appointment.

Repeat Violation

The center shall ensure that areas and equipment of the center, inside, are maintained in a clean, safe and operable condition.

In the 2 year old classroom, a knob was a missing from a play kitchen exposing a sharp screw.
Plan of Correction: Facilities were on site responding to work orders. Knob was removed.

Repeat Violation and Systemic Deficiency

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

The laundry room door located in the two year old classroom was not locked, sanitizing solution and paint were within reach of children.
Plan of Correction: Laundry Door is heavy duty. Children are unable to open. Facilities were on site, responding to work orders Lock was replaced, door locks immediately after shutting.

Each restroom area provided for children shall have sinks located near the toilets and that are supplied with running warm water that does not exceed 120 degrees Fahrenheit.

In the three year old classroom and the two year old classroom, the warm water temperature was approximately 137 degrees Fahrenheit.
Plan of Correction: Facilities were on site responding to work orders. Temperature was adjusted. Teachers always use both hot and cold water while washing hands.

The diapering area shall have a sink with running warm water not to exceed 120 degrees Fahrenheit.

The sinks located in the diapering areas in the Infant classrooms, used to wash infants hands, were approximately 134 degrees Fahrenheit.
Plan of Correction: Facilities were on site responding to work orders. Temperature was adjusted. Teachers always use both hot and cold water washing hands.
