Inspector Notes
An unannounced monitoring inspection was conducted today from 9:12am-10:00am with the provider on site. There were12 children in care with 3 caregivers. A sample of 2 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys, coloring shapes at the table and sleeping. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, first aid kit, radio, flashlight, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

5Evidence:
1. Household Member #3 had a TB test or screening dated 9/14/2021 and had not been updated every two years.
Evidence:
1. Caregiver #3 (start date 10/04/2022) did not have documentation of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. A bottle of Bruce Cleaning Spray, Comet Cleaner and Clorox Wipes were on the kitchen Counter and were accessible to children in the kitchen.
Evidence:
1. One unused outlet was not covered in the kitchen, two unused outlets were not covered in the playroom and one unused outlet was not covered by the door under the coats.
Evidence:
1. An Aquaphor ointment was on the bathroom counter that children use and was accessible to children.
