During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 03/12/2026, as a part of the licensure period. The on-site inspection began at 12:00 pm and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 13. The inspector reviewed 5 children?s records and 2 caregiver/household member(s) records. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to have employees undergo a background check every five years.

The most recent central registry check for staff #2 was overdue by seven months.
Plan of Correction: Provider will contact central registry office to request results.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were in the unlocked cabinet below the sink in the restroom the children use. There was an aerosol air freshener on a shelf at child level in the day care room.
Plan of Correction: Chemicals were moved during this inspection.

Children's records are required to contain the name, phone number, and address of two emergency contacts and parental authorization for emergency medical care should it be needed.

The record for Child #1 was missing the address for one of the two required emergency contacts. The child was present during the inspection and had been in care for a month. The record for child #5 did not contain emergency medical authorization.
Plan of Correction: Provider will request information from parents.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.

The record for child #3 did not have a parent?s signature for an annual review of the emergency contact information. This was overdue by two years.

Documentation of additional immunizations is required once every six months for children under the age of two years. '
