Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/15/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 11:00 and ended at 12:00. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 22. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Medication authorization expires every 10 days unless form is signed by a physician.

The provider has an emergency medication that has an authorization from the parent for a year but no authorization from the physician.
Plan of Correction: I will ask the parent for the medication authorization from the physician,

Medication must be in the original container with the prescription label with the name of the child, the name of the medication and the dosage needed.

The provider has two medications that are missing the original box and prescription label.
both medications are emergency medications.
