Inspector Notes
An unannounced renewal inspection was conducted on 9/19/2023 from 2:10pm to 4:18pm. There were 30 (4 months to School-Age) directly supervised by 7 staff. The physical plant, 2 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping and waking up from naps, participating in group and individual activities, and interacting with staff. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
5Evidence: observed that there was a wood border, loose and coming away from the wall, causing an entanglement and pinching hazard.
Evidence: observed that diaper ointment in the Twos classroom was not labeled with the child's name.
Evidence:
1) observed that the first aid kit on the lowest level was missing tweezers, a triangular bandage, thermometer, and a first aid manual.
2) observed that the first aid kit on the main level was missing 2 triangular bandages and a thermometer.
Evidence: reviewed 7 children's injury reports and found that 3 did not have documentation of time and 1 did not have the date parents' were notified of the injury.
Evidence: reviewed 5 children's records and found that Child 3 only had 1 emergency contact.
