Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.

The immunization record for child A was not signed. The physical and immunization record for child C was not signed.

REPEAT VIOLATION

The center shall ensure that areas and equipment of the center, inside and outside, shall be maintained in safe and operable condition.

1.) Three trikes were observed to be rusted with peeling enamel.
2.) One trike seat was cracked and posed a pinching hazard.
3.) One trike seat was broken leaving sharp edges exposed.
4.) Rust was present on the metal triangle pieces attaching the swing seats to the swing chains on the school-age playground swing bay.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

The allergies, sensitivities, and dietary restrictions lists in each classroom are not current. 1. The children documented on the allergy list dated 8/22/25 are no longer enrolled in the program. 2. The allergy list in the 3's and 4's classroom is dated 5/6/2025.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child A, enrolled for one month, was missing the following required components: Information regarding the child's guardian. One of the two required emergency contacts. The record for Child B, enrolled two months, was missing the following required components: The full name of two emergency contacts. The street address for two emergency contacts.
