Inspector Notes
This renewal inspection was initiated by licensing staff and concluded on 04/20/2023, between the hours of 11 am to 1 pm. There were 10 children present (33 points), ranging from infant to preschool-age, with 3 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication administration training; special care and emergencies; and nutrition. A total of 10 children records, 1 household record, and 5 staff records were reviewed.
Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

1. On the date of inspection (04/20/2023), Staff #2 had TB documentation on file that was dated - 03/22/2021.
2. Staff #2 was not on site on the date of inspection.

2. The last dated signature was from 09/10/2021. It is unclear when the current signature was acknowledged.
