Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 04/20/2023, between the hours of 11 am to 1 pm. There were 10 children present (33 points), ranging from infant to preschool-age, with 3 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication administration training; special care and emergencies; and nutrition. A total of 10 children records, 1 household record, and 5 staff records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-180-A
Based on review, 1 of 5 staff files indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis Screening (TB) form (in accordance with the requirements in 8VAC20-800-170) every two years.

Evidence -
1. On the date of inspection (04/20/2023), Staff #2 had TB documentation on file that was dated - 03/22/2021.
2. Staff #2 was not on site on the date of inspection.
Plan of Correction: Updated documentation will be obtained.
Standard 8VAC20-800-780-B
Based on review, 1 of 10 children files indicated that annually, the Provider reviewed with the parent the emergency contact information (required in 8VAC20-800-60 B 2) to ensure the information is correct, but did not obtain a completed signed acknowledgment of this review.

Evidence -
1. On the date of inspection (04/20/2023), the signed acknowledgement of Child #3's annual review was missing the date of signature.
2. The last dated signature was from 09/10/2021. It is unclear when the current signature was acknowledged.
Plan of Correction: The parent will date signature.
Standard 8VAC20-800-90-C
Based on review, 1 of 10 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (04/20/2023), an immunization page for Child #8 was dated - 08/22/2022.
2. On the date of inspection, based on information on file, Child #8 was under the age of two years.
Plan of Correction: Updated documentation will be obtained.
