Inspector Notes

A monitoring inspection was initiated on 12/11/2023 and concluded on 12/11/2023 from 4:00 PM to 4:40 PM. There were eight children present, ranging in ages from three to five, with one staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of four children?s records and the two staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation, the center failed to post the license and most recent inspection in a conspicuous place on the licensed premises.

Evidence:

An inspection was conducted of the facility. The license and inspection were not posted at the entrance and were not posted in the library where the children in the aftercare were located.
Plan of Correction: The license and inspection report will be posted in a prominent location.
Standard 8VAC20-780-160-C
Based on record review, the center failed to obtain updated tuberculosis (TB) screening every two years from the date of the previous screening.

Evidence:

1. The record for staff 1 documents the previous TB screening as 2/26/21. The current TB screening is dated 9/2/23.
2. The record for staff 2 documents the most current TB test is dated 6/27/22.
Plan of Correction: Staff 2 will obtain a TB screening.

Based on record review, the center failed to obtain the name, address and phone number of two designated people to call in an emergency if an parent cannot be reached.

Evidence:

Both emergency contacts for child 1 were listed as the child's parents.
Plan of Correction: The record for child 1 will be updated with two emergency contacts that are not the parents.
