During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 3, 2024 and completed on December 3, 2024. The on-site inspection began at 3:00 p.m. and ended at 4:10 p.m. The inspector reviewed compliance in the areas listed above. There were six children present and two staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included a tour of the facility, observations, interviews, and record review. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached and documentation of viewing proof of the child's identity and age.
The record for Child #5 did not contain two emergency contacts that were not the parents.
The record for Child #2 did not contain documentation of viewing proof of the child's identity and age.
Plan of Correction: The proof of birth and emergency contacts will be obtained from the parents of both children. In the future children will not be allowed to begin at the school until the records are reviewed to ensure all documents and information are complete.
