During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on October 15, 2025 and completed on October 15, 2025. The on-site inspection began at 10:00 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were 21 children present and four staff. The inspector reviewed five children?s records and four staff records on-site. The licensee provided addition record documents October 16, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunizations shall be signed by a physician, his designee, or an official of a local health department.
The record for child #4, who has been at the facility for eight weeks, does not have a signed immunization record.
Plan of Correction: Signed immunizations will be obtained for child #4. In the future the administrator will ensure all immunization records are signed by the physician.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Staff #1 and staff #2 have worked for the licensed preschool for six and half months. They had not completed the orientation course as of the time of the inspection. Staff #3 has worked for the licensed preschool for six and half months and there is no documentation of completion of the Virginia Department of Education-sponsored orientation course.
Plan of Correction: Both Staff #1 and Staff #2 had completed all but one topic. They completed that on 10/15/2025 and the certificates were forwarded to the licensing inspector. The certificate will be obtained from Staff #3 and added to the staff record.
Standard 8VAC20-780-240-I
Documentation of orientation training shall be kept by the center in a manner that allows for identification by individual staff member, is considered part of the staff member's record, and shall include: Name of staff; Training topics; Training delivery method; The entity or individual providing training; and The date of training.
Staff #1, staff #2, and staff #3 did not have documentation of completing orientation.
Plan of Correction: Documentation will be added to each staff file of orientation.
Standard 8VAC20-780-60-A
Each child's record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
The center has not requested the address for the emergency contacts this school year.
Plan of Correction: Addresses will be obtained for all children's emergency contacts. The initial documents to be completed by parents will be updated to include emergency contact addresses.
