Inspector Notes

An unannounced monitoring inspection was conducted on June 26, 2024 from 9:45 a.m. to 12:00 p.m. There were 25 children present, ranging in ages from six months to five years of age under the supervision of seven staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed during outdoor play, transition time and preparing for lunch. Lunch is brought from home.

A total of six child records and two staff records including one new staff were reviewed. One medication, allergy care plan and authorization were reviewed. Emergency drills were reviewed and up to date.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation and interview with staff, the center failed to request out of state central registry background check within 30 days of hire for staff that have resided outside of Virginia in the previous five years. Evidence: Staff #2, start date 4/29/2024, indicated out of state residence in West Virginia in the previous five years. There was no documentation of an out of state West Virginia central registry background check in the record.
