Inspector Notes

An unannounced monitoring inspection was conducted from 11:30 am - 1:00 pm with staff. There were 119 children, ranging from four months to 5 years of age, present with 19 staff supervising. Children were observed playing, preparing for lunch, eating lunch, preparing for nap and napping. Lunch served today was: chicken and rice, mixed vegetables, mixed fruit and milk. Five children's records and five staff records were reviewed. Five medications and authorization forms were reviewed and in compliance. The emergency drill log was reviewed and in compliance. Standard 22.1-289.035-B-4
Based on review of five staff records, it was determined that out of state background checks were not completed as required. Evidence: The record for staff #2, (date of hire: 9/22/2021) who indicated they lived out of state in the past five years, did not contain documentation of an Out of State Sex Offender Registry check or an Out Of State Criminal History Name Check prior to employment.
Plan of Correction: Staff stated they have already requested the missing background checks.
Standard 8VAC20-780-510-L
Based on observation, and staff interview, not all medication was stored in a locked place using a safe locking method that prevents access by children. Evidence: two emergency medications were stored in a plastic bag inside a backpack hanging on a hook near the exit door to the playground in the two's class.
Plan of Correction: Medication was moved to a locked storage bag during the inspection.
Standard 8VAC20-780-60-A
Based on review of five child records, it was determined that not all records contained all required information. Evidence: the record for child #2 did not contain the name, address or phone number for a second emergency contact or documentation of health information as required by 8VAC20-780-140. The records for child #1 and child #4 did not contain documentation of health information as required by 8VAC20-780-150.
Plan of Correction: Will ask the parents for the missing information.
