Inspector Notes

An unannounced complaint inspection was initiated on 8/9/2022 to investigate a self--eport received by the department related to Staffing and Supervision and Special Care Provisions and Emergencies. As a result of this inspection, and based on evidence gathered through interviews and documentation review, violations were issued to the facility. An exit interview was conducted on 8/24/2022.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Wednesday, 8/31/2022. Standard 8VAC20-780-340-A
Based on interviews and review of documentation, the facility failed to ensure that when staff are supervising children, they shall always ensure their care, protection and guidance. Evidence: On 7/26/2022, between approximately 5:30-6:00 pm, staff #1 was vacuuming the floor under a table in the school age classroom. Child #1 (age 5) was under the table and their hair was sucked up into the vacuum. Staff #1 stopped the vacuum when they heard the child scream, pulled the child?s hair out of the vacuum, and consoled the crying child.
Plan of Correction: Staff will wait to vacuum after all children have left the classroom

Based on interviews and review of documentation, the facility failed to maintain a written record of children?s minor injuries on the day of occurrence. Evidence: on 7/26/22 between approximately 5:30-6:00 pm, child #1 received a minor injury and an accident report was not written on the day of occurrence.
Plan of Correction: Staff will be sure to write up incident report same day and have ready for parents at pickup.
