An unannounced monitoring inspection was conducted on 12/9/2022 to investigate a self-reported incident regarding Care, protection and guidance. Interviews were conducted and documentation was reviewed. A violation was issued as a result of this inspection. An exit interview was conducted on 12/19/2022. Based on interviews, and review of documentation, it was determined staff did not always ensure the care and protection of all children. Evidence: Interviews revealed that on 11/21/2022, infant #1 (5 months old) was inadvertently fed another infant's bottle containing formula. Both bottles were labeled and dated, but similar in appearance. Staff stated that infant #1 brought four bottles to the center, drank three bottles, and had two full bottles left at the end of the day. Documentation revealed that infant #1 was fed three times that day.
