Inspector Notes

An unannounced monitoring inspection was conducted from 1:45 - 4:15pm with the director. There were 136 children, ranging from three months to 11 years of age, present with 20 staff supervising. Infant care was observed, and older children were observed napping, waking, playing in centers, eating snack and playing outside on the playground. Snack served today: apple slices, cheese cubes and crackers. Five child records, five staff records, and two medications were reviewed along with the emergency drill log, daily attendance, written allergy list, menu, and required posted information. Based on review of five staff records, it was determined that out of state background checks were not completed as required. Evidence: the record for staff #5 (date of hire: 3/22/2023) who indicated they lived out of state in the past five years, did not contain documentation that an Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment for all states in which they have resided, or an Out of State Sex Offender Registry search and Out of State criminal history name check completed prior to their first day of employment.
Plan of Correction: Will request out of state criminal history, sex offender, and child abuse and neglect searches.
