63.2 Child Abuse & Neglect

During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 4/15/2025 and completed on 4/24/2025 in response to a self-report received by the licensing office on 4/11/2025 relating to Forbidden Actions. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/13/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
1. On 4/8/2025, Child A was instructed to go to the table, Child A yelled and refused telling staff ?NO?. Staff A was then seen grabbing Child A (age 5 years) roughly by their arm to pull them up.
2. On one occasion, Staff A called out to Child B who was running in the classroom. Staff A then pulled Child B by the ponytail to stop the child from running and lowered the child to sit next to them all while still pulling the child?s ponytail.
Plan of Correction: Staff A has been terminated. All staff were retrained on appropriate child guidance during staff meeting.
Standard 8VAC20-780-410-1
Roughly handling a child is a forbidden action.

1. On 4/8/2025, Child A was sitting on the floor, Staff A was seen grabbing Child A roughly by their arm and pulling the child up by their arm from the floor.
2. On one occasion, Staff A was observed pulling Child B?s (Age 4 years) ponytail and lowered the child to sit next to them by pulling the child?s ponytail.
Plan of Correction: Staff A has been terminated. All staff were retrained on appropriate child guidance during staff meeting.
