An unannounced monitoring inspection was initiated and completed on 9/22/2025 in response to a self-report received by the licensing office on 9/16/2025 relating to behavioral guidance. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
