Inspector Notes

An unannounced monitoring inspection was initiated and completed on 3/13/2026 in response to a self-report received by the licensing office on 3/10/2026 relating to Staffing and Supervision. The inspector reviewed 1 children?s record on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 3/6/26, In the Pre-K classroom, Staff A, B and C were caring for 27 children. At approximately 3:20pm, the Pre-K staff and children went outside to the playground. During this time, Child A (4 years) was left in the Pre-K classroom unsupervised for 5-7 minutes. Staff D, who was walking by, discovered the child inside of the Pre-K classroom alone at the table. Approximately 2 minutes after child A was discovered by staff D, Staff C had already gone inside to retrieve the child from the classroom. By this time the child had already gotten returned to their class outdoors.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 3/6/2026, Child A (4 years) was left in the Pre-K classroom unsupervised and not within sight and sound supervision for approximately 5-7 minutes until discovered by another staff member walking by the classroom.

Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center's policy is that teachers perform name-to-face checks on their Child Supervision Records to ensure children are accounted for during the day. Teachers must look at each child's face to verify they are checking the correct child on their list. Staff A did not look at the face of Child A before marking the child accounted for, therefore the child was left unsupervised in the classroom for approximately 5-7 minutes.
