Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:00am-12:30 pm. There were a total of 32 children with five supervising staff members present. Children were observed during story time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard 8VAC20-780-160-A
Based on record review, each staff member did not submit documentation of a negative tuberculosis screening at the time of employment.
Evidence:
1. Staff #1's record (Date of hire-08/01/2022) had documentation of a negative tuberculosis screening dated 08/24/2022. This date is after the time of employment.
Plan of Correction: TB screenings will be submitted at the time of employment of within the last 30 days of the date of employment.
Standard 8VAC20-780-240-A
Based on record review, and interview, the Virginia Department of Education sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. Staff #2's record (Date of hire-09/07/2022) did not have documentation of the Virginia Department of Education sponsored orientation course.
2. Staff #5 confirmed that Staff #2 did not complete the required orientation training.
Plan of Correction: The staff member will complete the Virginia Department of Education sponsored orientation course.

Based on record review, required documentation was not maintained of shelter-in-place drills.
Evidence:
1. The Shelter In Place drills documentation log was missing the method used for notification of drill, the number of staff participating, and any special conditions simulated.
Plan of Correction: Shelter in place documentation will be documented on the model form provided by the Virginia Department of Education for compliance.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #4's record (Start date-08/29/22) did not contain documentation of two addresses of two emergency contacts.
