Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:00am-12:30 pm. There were a total of 32 children with five supervising staff members present. Children were observed during story time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
4Evidence:
1. Staff #1's record (Date of hire-08/01/2022) had documentation of a negative tuberculosis screening dated 08/24/2022. This date is after the time of employment.
Evidence:
1. Staff #2's record (Date of hire-09/07/2022) did not have documentation of the Virginia Department of Education sponsored orientation course.
2. Staff #5 confirmed that Staff #2 did not complete the required orientation training.
Evidence:
1. The Shelter In Place drills documentation log was missing the method used for notification of drill, the number of staff participating, and any special conditions simulated.
Evidence:
1. Child #4's record (Start date-08/29/22) did not contain documentation of two addresses of two emergency contacts.
