Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:00am-12:15pm. There were a total of 30 children with six supervising staff members present. Children were observed during water play, toy play, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard 22.1-289.035-B-4
Based on record review and interview, the center did not obtain a copy of the results of an out of state criminal history record information check from any state in which the individual had resided in the preceding years prior to employment.
Evidence:
1.Staff #1's record (Date of hire-06/17/2024) contained a Sworn Disclosure Statement that stated that the individual had lived in the state of Massachusetts in the preceding five years. Staff #1's record did not contain documentation of a criminal history records check for the state of Massachusetts.
2. Staff #6 confirmed that the out of state criminal history record check was completed incorrectly.
Plan of Correction: The center will conduct the correct out of state criminal background check for the staff member and document the record.
Standard 8VAC20-780-130-E
Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #3's record (Start date-01/04/2023) did not contain documentation of additional immunizations once every six months for children under the age of two years. Documentation of the last immunization record was dated 12/15/2022 when the child was 10 months old.
Plan of Correction: The center will obtain a copy of the current immunization records for the child and document the record.

Based on record review, each child did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child #1's record (Start date-04/02/2024) did not have documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Plan of Correction: The center will obtain a copy of the child's current physical examination.

Based on record review, staff records did not contain required information.
Evidence:
1.Staff #2's record (Date of hire-01/24/2024) did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, orientation and training.
Plan of Correction: The center will obtain documentation of the staff member's degree or copy of transcripts to document the record.
