Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 10:00am-12:15pm. There were a total of 30 children with six supervising staff members present. Children were observed during water play, toy play, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
4Evidence:
1.Staff #1's record (Date of hire-06/17/2024) contained a Sworn Disclosure Statement that stated that the individual had lived in the state of Massachusetts in the preceding five years. Staff #1's record did not contain documentation of a criminal history records check for the state of Massachusetts.
2. Staff #6 confirmed that the out of state criminal history record check was completed incorrectly.
Evidence:
1. Child #3's record (Start date-01/04/2023) did not contain documentation of additional immunizations once every six months for children under the age of two years. Documentation of the last immunization record was dated 12/15/2022 when the child was 10 months old.
Evidence:
1. Child #1's record (Start date-04/02/2024) did not have documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1.Staff #2's record (Date of hire-01/24/2024) did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, orientation and training.
