Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on July 25, 2025, as a part of the licensure period.The on-site inspection began at 9:30am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation

The center shall obtain documentation of additional immunizations once every six months for the children under the age of two years.

Documentation of additional immunizations were not obtained once every six months for Child #1 and Child #2. Child #1, 14 months old, did not have documentation of additional immunizations in 12 months. Child #2, 19 months, did not have documentation of additional immunizations in 12 months.
Plan of Correction: The parents have been contacted to obtain updated immunization records. Management will ensure that additional immunizations are obtained once every six months for children 2 and younger by doing a weekly check of children's records.

Documentation of orientation training shall be kept by the center in a manner in a manner that allows for identification by individual staff member, is considered part of the staff member's record and shall include required information.

Documentation of orientation for Staff #2 was missing from the record. Staff #2 has been employed for five months.
Plan of Correction: Staff completed staff orientation the first week of employment. Management will ensure that documentation of staff orientation is placed in the staff record by using a checklist.

Cots shall be identified for use by a specific child.

The cots located in the Toddler's classroom were not identified for use by a specific child.
Plan of Correction: The cots will be labeled today and moving forward, staff will ensure cots are labeled for specific use by a child by completing a classroom checklist.
