Inspector Notes

An unannounced, on-site renewal inspection was initiated on 05/19/2025 and completed 05/21/2025. The onsite inspection began at 10:15 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 145 children present and 36 staff. The inspector reviewed 10 children?s records, 10 staff records and 4 applicant records on-site.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Cleaning materials and insecticides/pesticides shall be stored in areas physically separate from each other.

There was an insecticide and a disinfectant stored on the same shelf, in the same cabinet in the outside restroom.
Plan of Correction: The insecticide was moved to a storage area during the inspection.
