During the inspection, the licensing representatives reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused monitoring inspection was initiated and completed on 10/17/2025. The Department issued a Notice of Intent to Deny Renewal of the License on 08/11/2025 , as a result of high-risk violations. The on-site inspection began at 9:15 am and ended at 12:16 pm. The inspector reviewed compliance in the areas listed above. There were 137 children present and 39 staff. The licensing representatives reviewed 5 staff records on-site.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

There was a tricycle in use on the outdoor playground that was missing the plastic pedals exposing the medal pegs, creating potential for impalement.
Plan of Correction: The tricycle was removed from the area. Teachers will be reminded of the playground check.
Standard 8VAC20-780-500-A
Staff shall wash their hands before and after a diaper change and before preparing or serving food or beverages.

Staff 2 did not wash their hands before preparing a bottle. Staff 6 did not wash their hands before a diaper change.
Plan of Correction: The staff will be reminded of the requirements and proper administrative actions will be taken. Will post handwashing steps in all classes.

The licensee shall post the Notice of Intent in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.

The Notice of Intent, issued on 08/11/2025, was not posted at the facility.
