Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12-9-2024 and completed on 12-9-2024. The on-site inspection began at 2:35pm and ended at 4:15pm.. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregivers; the point total was 2.
The inspector reviewed 4 children?s records, 1 provider record and 1 household member record on site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12-17-2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-180-A
Each provider and adult household member shall have a current report of Tuberculosis Screening (TB) form, every two years from the date of the first screening. Household member #1 TB screening is overdue by 6 months.
Plan of Correction: Household member #1 has scheduled an appointment for a TB screening the
week of 12-16-2024 and will be completed by 12-23-2024.
Standard 8VAC20-800-280
Poisonous substances shall be stored in an area inaccessible to children. Poisonous hygiene liquids and tablets were in the bathroom stored in unlocked cabinet, on the sink and on the bathtub rim and were accessible to children. These items were labeled "keep out of reach of children" and labeled with at least one other statement "warning", "flammable" and "hazardous".
Plan of Correction: Hand soap located on the bathroom sink and/or body wash located on bathtub rim
has been replaced with products that DO NOT read "Keep out of reach of children"
or other statements "warning", "flammable" & "hazardous" or has been placed higher from reach of children. In addition, a clock has been placed on the bathroom cabinet.

Children's records are required to contain all required information. .

Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water before eating. Child #4 was observed eating and did not wash their hands prior to eating.

Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated. The menu posted was dated for the week of 12-2-2024.
