Inspector Notes

An unannounced monitoring inspection was conducted on 4/17/24 from 3:30 p.m. to 4:40 p.m. with the site director. There were 11 children in care, ranging in age from five to 12-years-old, supervised by two staff. The children were observed in free play. Four child records and three staff records were reviewed. The Center has three staff with current certification in CPR and First Aid. Five medications and authorization forms were reviewed, and the center has staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Health Inspection on file was dated 11/2/23. Areas of non-compliance are identified in the Violation Notice. Based on a review of three staff files, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children.

The record for Staff # 2, with a start date of 1/24/24, contained documentation of a tuberculosis screening dated 1/26/24.
Plan of Correction: Staff member was not in contact with children prior to receiving a TB test. Dates on file have been updated and staff will not be in sites until after TB test received.
