An unannounced inspection was conducted today.
There were 21 children (ages 3-5 years) present with six staff.
The children were on a field trip to the library and working in centers.
Four child records and two staff records were reviewed.

Based on a review of staff files, the facility failed to ensure that staff who have lived in another state in the last five years, complete an out-of-state background check before the 30th day of employment.
Evidence:
