During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 02/06/2025 and completed on 02/06/2025. The on-site inspection began at 9:18am and ended at 11:58am. The inspector reviewed compliance in the areas listed above. There were 105 children present and 18 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/18/2025. A POC submitted after this date will not appear on the public website.
Violations
3The most recent national criminal background check for officer #1 was overdue by two years. The most recent national criminal background check for officer #2 was overdue by one year.

The most recent central registry background check for officer #1 was overdue by two years. The most recent central registry background check for officer #2 was overdue by one year.

Moving forward, these two officers have been added to staff
information sheet to prevent this oversight from happening again.
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
The screening for Staff #2 was completed three weeks after employment.
The screening for Staff #6 was completed 44 days before employment.
Staff #7 updated screening was due four months ago.
The screening for Staff #9 was completed nine days after employment.
results need to be in hand before the first day of employment.
Moving forward, all new employees will be required to submit TB screening before the first day of employment. The employee who needs to renew that has been notified to complete this ASAP.
Staff # 1, employed for two years and 6 months, did not have the Virginia Department of Education-sponsored orientation course.
Staff #2, employed for one year and five months, did not have the Virginia Department of Education-sponsored orientation course.
Staff #6, employed for one year, did not have the Virginia Department of Education-sponsored orientation course.
Staff # 9, employed for six months, did not have the Virginia Department of Education-sponsored orientation course. Staff #9 did not have a completed orientation training on file.
Moving forward, all new employees will be required to complete this within the first 30 days of employment.
