During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/11/2025 and completed on 09/11/2025. The on-site inspection began at 9:20am and ended at 12:16pm. The inspector reviewed compliance in the areas listed above. There were 120 children present and 19 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/18/2025. A POC submitted after this date will not appear on the public website.

Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor who meets all requirements.

Staff #11 was the only MAT trained staff in the building and had a MAT certification that expired 10 days ago. The center had emergency medication on site at the time of the inspection.
Plan of Correction: We are taking steps to have this staff member's MAT training renewed within the next 30 days. We also plan to have two additional staff AMAT trained.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

There were no staff on duty at the time of inspection who had current training in performing daily health observation of children.
Plan of Correction: We will train at least one staff in DHO and take steps to ensure this is renewed every three years.

Annual approval from the health department is required.

The center?s health inspection was three weeks past due.
Plan of Correction: We have contacted VDH and are awaiting their response with an inspection date.
