Inspector Notes

An unannounced monitoring inspection was conducted on February 23. 3023 from 10:00 a.m. to 11:10 p.m. There were 25 children present, ranging in ages from three to five years old, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to ensure that the medication authorization was available to staff during the entire time it is effective.
Evidence:
1. Medication #1 there was an allergy action plan signed by a doctor on file. There was no medication authorization form for Medication #1.
2. Staff #5 confirmed there was no medication authorization form for Medication #1.
