Inspector Notes

A monitoring inspection was conducted on April 11, 2023. There were sixty-six children present with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 2 of 5 children's records have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #2 (start date 11/17/20) did not have an address for the second emergency contact.
2. The record for child #4 (start date 4/3/23) did not have one parent's employment phone number.
Plan of Correction: Staff contacted parents during inspection and information was added to the children's records.
Standard 8VAC20-780-80-A
Based on observation and review, facility did not ensure 4 of 8 classrooms have a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance sheets in the following classrooms did not accurately reflect the number of children present in the classroom during inspection: toddler, K3, K3/4 and K4/5. Staff stated that some children had already left for the day and/or had not been signed in yet.
Plan of Correction: All staff will be retrained on attendance documentation. Director and Assistant Director will work on a system to properly document pick up.
