Inspector Notes

An unannounced inspection was initiated and completed on October 30, 2024 in response to a complaint received by the licensing office on October 28, 2024 relating to allegations of staffing/supervision. The inspector reviewed 1 child's record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #1 had not completed the course and had been employed for eight months.
Plan of Correction: If a staff has not completed the training within 90 days, the individual will be suspended until it is completed or terminated.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. On October 22, 2024, child #1 (age 22 months) received an injury to the elbow when staff #1 picked up the child by holding onto the child's hands and wrists and lifting the child 25" high to place him on a diapering table.
Plan of Correction: Staff were retrained on the proper way to pick up the children. Staff member is no longer employed.
Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. The center was aware on October 23, 2024 that child #1 sustained an injury that required medical treatment and the department was not notified by the center.
Plan of Correction: The center immediately reported the findings to the parent but did not report it online to the department. If anything occurs in the future, the incident will be reported to the department within 2 business days.
