During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/14/2025. The on-site inspection began at 12:50 pm and ended at 4:15 pm.The inspector reviewed compliance in the areas listed above. There were 76 children present and 13 staff. The inspector reviewed 7 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
The records for child #1 and child #6 contained immunization records which were not stamped or signed by a physician, his designee, or an official of a local health department.
Plan of Correction: The program director stated that she would contact the parents of child #1 and child # 6 tomorrow and require that they obtain a physician's signature on their child's immunization record.
Standard 8VAC20-780-510-E
The center's procedures for administering medication must Include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. For long term administration more than 10 days the procedures must include written authorization from the child's physician and parent.
Seven medications maintained on site for long term emergency use did not have a physician's written authorization form allowing for administration longer than 10 days.
Plan of Correction: The program director stated that she would contact the parents of children with emergency medication kept on site tomorrow and require that they obtain a physician's signature on their child's long term medication authorization form.
