Inspector Notes

An unannounced monitoring inspection was conducted on September 22, 2023 from approximately 9:45 am to 11:00 am. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, preventing the spread of disease and nutrition. The provider record, 1 household member record, and 2 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 4 children in attendance for a total of 12 points. The children were observed playing in the playroom and eating a snack while the inspector was on the premises.

Violations were issued.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 10/10/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on record review, and interview, the provider failed to ensure applicants and adults involved to day-to-day operations or living in the family day home undergo background checks every five years.
Evidence: The provider was unable to locate the results of the Central Registry background check for themself and a household member. The Central Registry results in the records were dated 11/22/2017. Central Registry checks were due on or before 11/22/2022. The provider stated that the background checks were conducted but that they were not able to be located.
