Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. Areas of non-compliance were found with the supplemental subsidy health and safety requirements. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Based on records reviewed, it was determined that 3 of 4 staff are not currently certified in CPR.
Evidence:
1. Staff #1 - CPR certification expired 9/10/21.
2. Staff #3 - CPR certification expired 6/11/21.
3. Staff #4 - There was no CPR certification on-site however, according to the center's renewal application dated 9/14/21, the CPR certification expired 8/17/19.
Evidence:
1. Staff #1 - CPR certification expired 9/10/21.
2. Staff #3 - CPR certification expired 6/11/21.

1. Staff #3 - First Aid certification expired 4/10/21.
2. Staff #4 - There was no First Aid certification on-site however, according to the center's renewal application dated 9/14/21, the First Aid certification expired 8/17/21.

2. Staff #3 - The annual Virginia Health + Safety Training certificate was dated 5/16/19.
3. Staff #4 - There was no documentation of the Subsidy Pre-Service training being completed within 90-days of the date of hire 10/29/18, and there was documentation of the subsequent annual Virginia Health + Safety Training certificates.
